(1) For the purpose of Section 17 of the Act, a written application to the relevant agency or officer requesting to give a petition or appeal. When giving, it should be given in the format as per schedule-11.
(2) A copy of the order or decision of the court shall also be attached to the application given in accordance with sub-rule (1).
